Accident or Incident Procedure

Introduction

An accident is an unplanned, uncontrolled event, which causes, or could cause injury, damage or loss. In most cases accidents can be avoided and it is our intention to prevent as many as possible.

Pupils

  • Any pupil complaining of illness or who has been injured during lesson time should be sent to a qualified first aider to inspect and where appropriate administer first aid. The pupil should not be sent to the office on their own but should be accompanied by either another pupil or a member of staff. If the first aider feels the pupil needs to go to hospital, go home or a parent is called to the school, the Headteacher or Member of SLT must be informed immediately.
  • All accidents, ailments and first aid treatment must be recorded in the accident book and in the school’s pupil mamangement system.
  • Accidents / incidents of a more serious nature including incidents of violence and aggression are recorded on CS2 Accident forms obtainable from the school office. The CS2 forms are signed by the Headteacher and recommendations implemented where possible to prevent reoccurrence.
  • HEAD INJURY – parents are always contacted if a pupil sustains a head injury. A first aid slip is issued to parents to ensure they are aware of the incident.
  • In the event of a serious incident an ambulance will be called immediately by the school office. A first aider will decide whether the pupil should be moved. If there is a suspected fracture to the legs, hips, ankles it is safer not to move the pupil unless they are in imminent danger. A staff member will accompany the pupil if the parents have not yet arrived at the school, they will be asked to go immediately to the hospital.

Employees

  • An injury sustained at work by an employee should be entered into the management information system. An injured member of staff or other supervising adult should not continue to work if there is any possibility that further medical treatment is needed. The member of staff or other supervising adult should seek medical advice without delay.

CS2

  • Completed CS2s will be entered on to the Lewisham Online Accident/Incident reporting system as soon as possible after the incident occurs, together with witness statements and related documentation if available.

Investigation

Depending on severity of incident/injury, the head of school will initiate an investigation. Witness statements, photographs and any other relevant information will be gathered and used to inform the process. Relevant risk assessments may need to be reviewed.

Reporting of Injuries Diseases, Dangerous Occurrences

The RIDDOR regulations put a duty on the employer to report certain categories of accidents/ incidents and occupational diseases to the Health and Safety Executive (HSE). You have to report if

  • All deaths caused by workplace accidents
  • Specified injuries caused by workplace accidents
  • Occupational diseases
  • Exposure to carcinogens, mutagens and biological agents
  • Specified injuries to workers
  • Dangerous occurrences
  • Gas incidents
  • Over-seven-day incapacitation of a worker. Accidents must be reported where they result in an employee being away from work, or unable to perform their normal work duties, for more than seven consecutive days as the result of their injury
  • Over-three-day incapacitation. Accidents must be recorded but not reported where they result in a worker being incapacitated for more than three consecutive days.
  • Non-fatal accidents to non-workers (pupils, students, members of the public, parents, visitors, volunteers). Must be reported if they result in an injury and the person is taken directly from the scene of the accident to hospital for treatment for that injury.

This procedure will be review every two years by the resources committee or sooner if required.
